Feedback, Compliments & Complaints
Your experiences help us celebrate what we do well and find ways to improve our services. We welcome compliments, feedback, suggestions and complaints. You can use this form, call us on 0411 802 325, email admin@cornerstonetherapies.com.au, or talk with us in person.
Your rights
You can give feedback or make a complaint anonymously. If you do, we won’t be able to contact you about the outcome.
Raising a concern or complaint will not affect the support you or your child receive.
You can have a family member, friend or independent advocate help you or speak on your behalf. To find a free disability advocate, visit the Disability Advocacy Finder.
Your information is kept confidential and stored securely. We only share it if the law requires it or if it’s needed to keep someone safe.
What happens next
We will acknowledge your complaint within 2 business days, involve you in resolving it, keep you updated, and explain our decision and your options if you’re not satisfied. If your complaint involves harm, abuse or neglect, we will also manage it under our incident management procedures and report it where required.
NDIS participants
If you’re not satisfied, or would rather not raise it with us, you can contact the NDIS Quality and Safeguards Commission on 1800 035 544 (Monday to Friday, 9am–5pm), TTY 133 677, or online at www.ndiscommission.gov.au/complaints. We are happy to help you contact them.
